Join Harborview Medical Center's EDWARD THOMAS HOUSE MEDICAL RESPITE AMBULATORY and ALLIED CARE SERVICES as a per diem/hourly Healthcare Specialist (HCS) (ARNP; Nurse Practitioner; or PA-C, Physician Assistant).RESPONSIBLE TO: Medical Director and Health Services Manager The Edward Thomas House Medical Respite is a unique medical respite harm reduction program that provides shelter care with embedded nursing, mental health, and case management services to an at-risk homeless population. Medical Respite is a part of the Healthcare for the Homeless Network and is supported by funds from multiple sources including King County Public Health, MIDD, and local hospitals. The Edward Thomas House admits patients referred from all King County area hospitals 7 days a week and operates 24 hours a day, 365 days a year. The program is operated by Harborview Medical Center (HMC) at an off-site location next to the Harborview emergency department. As the region's only Level I Trauma center, Harborview Medical Center (HMC) is well known for innovations and excellence in trauma care and its centers of emphasis: Trauma, Burn, Neurosciences, AIDS/STD, Center for Advanced Reconstruction and Rehabilitation (CARER), and mentally-and medically- vulnerable populations. In addition to the centers of emphasis, HMC serves a mission population for King County. JOB SUMMARYThe Healthcare Specialist (HCS) ARNP; Nurse Practitioner; or PA-C, Physician Assistant) functions independently in a staff position to implement the role of expert practitioner, consultant, educator and researcher within the specialized area of patient care. The HCS possesses and exhibits a high degree of knowledge, skill and competence in the provision of health care directly to the patient, family or community, or indirectly through guidance of care by other professional personnel. The HCS performs within the scope of licensure and in accordance with clinic, hospital and university policies/standards. MAJOR RESPONSIBILITIES
Perform admission histories and physicals on all patients admitted to Medical Respite
Provide a pain management plan and pain medication prescriptions for patients during their respite stay, unless pain management is arranged to be provided by non-respite providers
Assess and manage acute medical issues that arise during a patient's respite stay
Documents findings, reports and plans of care including needed patient education and appropriate follow up of care in the medical record.
Performs efficiently during emergency situations, using sound judgment and following established protocol.
Participates in an interdepartmental effort to facilitate a safe environment for staff and patient care.
Respond to questions and request regarding patient care from the RN and mental health teams
Participate in interdisciplinary meetings regarding patient care and facilitate discharge and clinic decisions that are required
Consult attending physicians and subspecialty providers when indicated for patient care
Coordinate care with provides involved in the care of medical respite patients
Review and finalize discharge medication list and provide prescriptions to last for 30 days post respite discharge
Complete a discharge summary for each patient leaving medical respite
Facilitate weekly team rounds when required
Assists in the training of various levels of health care professionals.
Utilizes appropriate lines of authority to maintain effective communication
